Conservative management is the modality of choice for acute lateral ankle ligament injuries, and operative treatment is reserved for special cases. Failure after strict rehabilitation may be an indication for surgery. Several operative options are available, including anatomic repair, anatomic reconstruction, and tenodesis procedures. Anatomic repair can be performed when the quality of the damaged ligaments permits. Anatomic reconstruction with an autograft or allograft should be considered when the torn ligaments are not adequate. Ankle arthroscopy is a useful adjunct to ligamentous procedures, performed at the time of repair to identify and treat intra-articular conditions that may be associated with chronic ankle instability. Tenodesis techniques are not recommended because of their suboptimal long-term results related to the modification of ankle and hindfoot biomechanics.Level of Evidence Level V, expert opinion. There are conflicting data with regards to the link between diet and the prostate cancer. The purpose of this study was to assess the association of dietary factors with incidence, prevalence and mortality rates of prostate cancer worldwide. We conducted an ecological study including 170 countries, whose data on incidence, prevalence and mortality rates of prostate cancer, dietary factors, and potentially confounding factors were available and collected in May 2020. Univariable and multivariable linear regression analyses were used. Consumption of nuts and seeds was inversely associated with incidence, prevalence and mortality rates of prostate cancer (β -0.7, P less then 0.001; β -2.1, P less then 0.001; β -0.1, P = 0.02; respectively). Intake of alcohol was associated with increased incidence, prevalence and mortality rates of prostate cancer (β 1.8, P less then 0.001; β 4.5, P less then 0.001; β 0.4, P less then 0.001; respectively). Consumption of processed meats was also associated with increased incidence and prevalence rates of prostate cancer (β 0.6, P = 0.003; β 2.8, P = 0.001; respectively). These data suggest that consumption of nuts and seeds have a protective effect against prostate carcinogenesis, progression, and metastasis, while alcohol and processed meat increase these risks.
To hypothesize the protective role of vitrectomy on the natural course of diabetic retinopathy.
Case report.
A 66-year-old lady with a history of type 1 diabetes mellitus since the age of 10 was referred for evaluation. She received laser treatment in her left eye for proliferative diabetic retinopathy 6 years earlier, while her right eye underwent vitrectomy for retinal detachment 17 years before. Diabetic retinopathy was found to be substantially asymmetrical, as the vitrectomized right eye had only mild macular edema without significant vascular abnormalities, whereas the left eye had a complete confluent panretinal photocoagulation with advanced atrophic macular changes. Other reasons for this asymmetry, such as axial length disparity, carotid circulation impairment, or history of branch retinal vein occlusion, were not identified.
Vitreous removal, performed for a reason other than diabetic retinopathy, may inhibit the retinal complications often associated with longstanding diabetes. Further clinical studies should be considered to ascertain such evidence.

To evaluate the efficacy of symmetric bilateral medial rectus recession in large-angle esotropic Duane retraction syndrome (DRS) with moderate to severe globe retraction.
In a retrospective study, medical reports of 30 patients with esotropia of equal or more than 20 prism diopters (pd) and moderate to serve globe retraction due to unilateral DRS who underwent symmetric bilateral medial rectus recession were reviewed. Age, gender, laterality, amblyopia, length of follow-up, pre- and postoperative measurements of primary position deviation, ocular ductions and severity of globe retraction and abnormal head posture were evaluated. A successful result was defined as decreasing esotropia to equal or less than 8 pd or equal or less than 8 pd of consecutive exotropia.
The mean age of patients at surgery was 13.7 ± 8.5 years old (range 3-38). The mean preoperative esotropia measured 28.9 ± 9.1 pd in distance and 25.7 ± 7.2 pd in near, which decreased to 4.9 ± 6.1 pd in distance and 3.9 ± 8.8 pd in near postoperatively. The mean bilateral medial rectus recession was 4.9 ± 0.9 mm (range 3-6 mm). The mean abnormal head posture improved from 19.1 ± 6.9 degrees (range 10-30 degrees) to 3.3 ± 4.7 degrees (range 0-15 degrees) postoperatively. At the last follow-up visit, 23 patients (76.7%) had a successful outcome.
In the large-angle esotropic DRS patients with moderate to severe globe retraction, symmetric bilateral medial rectus recession, can be conducted to successfully resolve primary position deviation and abnormal head posture.

To investigate ophthalmic side effects of taxanes and compare side effect frequency, requirement for cessation of taxane therapy, visual acuity outcome, and concurrent systemic effects between paclitaxel and docetaxel.
Patients taking taxanes at a single center from 1/1/2010 to 2/29/2020 were retrospectively reviewed for clinical characteristics, treatments, and concurrent systemic adverse effects.
Of 1918 patients, 22 (1.1%) experienced an ophthalmic side effect that came to the attention of an eye care provider. Saracatinib Mean age at presentation of the side effect was 62 years (median 66, range 23-82). The most common side effect was meibomian gland dysfunction in 5 (23%) patients, followed by cystoid macular edema in 4 (18%) patients and canalicular obstruction in 4 (18%) patients, followed by diplopia in 2 (9%) patients, and singular cases of lash alopecia, and blepharitis, among others. Lids/lashes as well as nasolacrimal duct adverse effects occurred more frequently with docetaxel therapy than with paclitaxel therapy.
